Avera MyPlan offers two plans with the option to add maternity, routine dental and vision coverage. Instructions to calculate monthly rates are on the bottom of the rate sheets.

Individuals and families living in South Dakota may apply for Avera MyPlan.

The preliminary rates are an estimated monthly premium (cost for the health plan) and not a guarantee you can purchase health insurance. Rates may be higher than calculated because of medical underwriting and the optional benefits selected. Some applicants may have exclusionary riders or coverage declined based on medical underwriting.
