It is the member's responsibility to verify that the provider is in the network before an appointment is made or services provided.  Again, if you are currently a member, access your provider directory through your member login.

Also note that providers are participating with Avera Health Plans only at the clinic sites listed. If you receive services at a different clinic site, the payment will be at the out-of-network benefit.
