The familiar environment of your own home is often the best place to receive care. Our Home Care Agency offers a wide variety of medical services and equipment for the home.

With the support of our team, patients of all ages can receive quality care at home, rather than at the hospital or long-term care facility. We work with you, your physician, family and friends to design the right care plan and deliver it in the comfort of your own home.

As part of our Total Care philosophy, patients of Home and Community Care can be assured of a seamless movement throughout our healthcare system. From outpatient services, to inpatient stays, and back to home. This approach enhances the patient's ability to communicate with his or her caregivers within the context of a complete care plan.

Our Home and Community Care offers the following services:

• Home health nursing
• Home health aide care
• Rehabilitative exercise in physical and occupational therapy
• Private Pay Services

Care giving teams are available 24 hours a day, seven days a week, and a registered nurse is just a phone call away. Services are provided on a per-visit or hourly basis, at a fraction of the cost of a hospital or nursing home stay. Most insurance companies, Medicare, Medicaid and veterans benefits will pay for home care services, or they can be paid for privately. Since all policies are different and many are complicated to understand, department personnel recommend calling for an appointment during which they will verify coverage, free of charge.
