Osceola Community Health Services provides professional care for clients of all ages. An individual plan of care is developed to meet each person's special needs. The agency provides a full range of health care and social services to the client and his family in familiar surroundings.

Foot Care Clinic:
You are welcome to attend any clinic in the county, if you call ahead and make an appointment with the contact person of that community.
If able, Please soak your feet prior to your visit.

Referrals
Referrals can be made to Osceola Community Health Services by medical professionals, social workers, family members, and clients. In-home evaluations are available free of charge. Clients need not be in the hospital for home care services to be initiated.

Osceola Community Health Services is a Medicare certified agency. The agency is governed by the Board of Health and Osceola Community Hospital Board of Trustees. Our office would be pleased to assist you with determining your funding source and will submit your claims for services.
Hours and Location
Home care services are available 24 hours a day.
Regular office hours are from 8:00 a.m. to 4:30 p.m., Monday through Friday.
