Patient and Family Centered Care

At SCCH&HC, we provide care that is respectful of and responsive to individual patient preferences and needs to ensure that patient values guide all clinical decisions. We place patients at the center of our services to provide care that is family-centered, coordinated, compassionate, and effective.

We realize that the term "quality care" means different things to each of us. For us, quality is part of each and every patient experience. To be truly healing and effective we believe that care must be delivered in a way that is sensitive to patients' concerns and comfort and actively involves patients and family members in shared decision making about their care. We continually monitor and seek to improve all processes to ensure that patients receive exceptional care.

Interpreter Services

Interpreter services are available to all patients free of charge. Interpreter services are available 24 hours a day, 7 days a week either in person or via our SpectraCorp Language Line.

Community Health Needs Assessment

In 2013, Sioux Center Community Hospital & Health Center Avera conducted a Community Health Needs Assessment in order to better understand the health concerns of our community and primary service area. While the CHNA is a federal requirement, conducting a needs assessment and resonding to those community health needs allows SCCH & HC to live their mission and make a significant impact in the community.
