Medical Clinics

In addition to our hospital services, we offer the health care of a full medical clinic, both in Sioux Center and the community of Hull for our patients' convenience. We work to provide services for the entire family, including obstetrics, pediatrics and care for older adults.

Patient Registration Form

Effective July 2, the medical clinic will be changing their computer software. We will be asking you to complete a NEW patient registration form and patient history form Please complete the form prior to your appointment. Also, we will be scanning both your insurance care and driver's license. Please bring both to your appointment.

 

Urgent Care Clinic

Our Urgent Care Clinic offers an evening and weekend solution for unexpected injuries and illnesses without an appointment. No appointments are necessary.
