Employment Benefits

BENEFIT ELIGIBILITY PAYMENT
Health Insurance   Coverage available on the first of the month following 30 days of employment. Employees must be regularly scheduled to work 40 hours per pay period.   Insurance is a major medical program. ADMH and employee share the cost of the premiums. Premiums are pre-tax.

Life Insurance   Coverage effective on the 91st day of employment. Employees must be regularly scheduled to work 40 hours per pay period.   ADMH pays 100 percent of the premium. Benefit provides one times employee's yearly salary.

Long-term Disability Insurance   Coverage available on the first of the month following 30 days of employment. Employees must be regularly scheduled to work 40 hours per pay period.   Employee pays 100 percent of the premiums. Insurance provides 24-hour protection against the loss of income resulting from a disability.

Dental Insurance   Coverage available after one (1) year of employment. Employees must be regularly scheduled to work 40 hours per pay period.   Insurance covers various dental procedures. ADMH pays 100 percent of premiums for employee coverage. ADMH and employee share the cost of premiums for family coverage. Premiums are pre-tax.

Vision Benefit   Available after one (1) year of employment. Eligible employees must be regularly scheduled to work 40 hours per pay period.   Covers a standard eye exam once every 24 months for one family member. ADMH pays reimbursement amount that is determined annually.

401(k) Plan   Available on January 1 or July 1 after one (1) year of employment. Employees must be age 21 or older who have worked 1000 hours in a qualifying year.   Benefit provides a retirement investment plan. ADMH makes a yearly contribution. Employees may also contribute on a pre-tax basis.

Supplemental Insurance   Coverage available on the first of the month following 30 days of employment. Employees must have regularly scheduled hours.   Supplemental insurance coverage includes a variety of policies. Employee pays 100 percent.

Tax Deferred Annuity (TDA)   Available upon employment. Employees must have regularly scheduled hours.   TDA is a tax-deferred retirement savings plan. Employee pays 100 percent.

Paid Time Off (PTO)   Accural begins upon employment; usage begins after 90 days of employment. Employees must be regularly scheduled to work 32 hours or more per pay period.   PTO is a bank of vacation, sick leave and holiday hours.

Extended Illness Time (EIT)   Accural begins upon employment; usage begins after 90 days of employment. Employees must be regularly scheduled to work 32 hours or more per pay period.   EIT is a bank of leave hours to be used in the event of an extended illness.

Bereavement Leave   Available to all employees upon employment.   Leave to be used in the event of a family member's death.

Shift Differential   Available to all hourly employees upon employment.   ADMH pays employee an additional amount per hour for working evening and night shifts.

Stand-By Pay   Available to all hourly employees upon employment.   ADMH pays employee for time spent in a stand-by status.

Flexible Spending Accounts   Available upon employment. Employees must be regularly scheduled to work 32 hours or more per pay period.   A pre-tax spending account employees can use to pay dependent care and health care expenses. Employee contributes 100 percent.

Tuition Assistance   Available after one (1) year of employment. Employees must be regularly scheduled to work 32 hours or more per pay period.   Tuition assistance for employees working to further their education.

Employee Store   Available to all employees upon employment.   Employee receives discount prices on various items in store catalog.

Fitness Center Membership   Available upon employment. Employees must have regularly scheduled hours.   Employee receives a discount on fitness center membership rates.
